HOW THE MINNESOTA AGING PATHWAYS CAN HELP YOU

The Minnesota Aging Pathways helps older Minnesotans and their caregivers find answers and connect to the services and support they need. We can help with Medicare, finding prescription drug cost assistance, forms and applications completion, long-term care counseling, finding resources in your community, caregiver support and much more. The Arrowhead Area Agency on Aging is a direct provider of Information, Referral and Assistance through Minnesota Aging Pathways. The services of Minnesota Aging Pathways are available over the phone. Call the Minnesota Aging Pathways at 800-333-2433, Monday through Friday 8:00 AM to 4:30 PM. A Minnesota Aging Pathways Specialist provides information and assistance counseling to support older adults successful aging in the Arrowhead!

The Minnesota Aging Pathways strives to provide accurate, comprehensive and timely information to help people plan well, live well and age well.  

The Minnesota Aging Pathways provides comprehensive Long Term Care Options Counseling services using current technology to provide the best service possible, with a high level of customer service and satisfaction as the number one priority.  

 

Staff and volunteers can help: 

  • People understand their rights and make informed decisions 

  • Make health insurance counseling services accessible to all people with Medicare, older adults, people with disabilities, people with prescription drug issues, people with end-stage renal disease and their family members and caregivers, and 

  • Provide information over the phone, in person and through outreach and education events. 

  • Find housing resources 

  • Find services for in the home 

  • Find caregiver support services 

  • Find or optimize your health insurance 

  • Save on prescriptions medications 

  • Evaluate and determine what services might be helpful for you to maintain your independence. 

 

Our staff can assist you in locating a provider, develop a plan for support in your community and they can also provide follow up in your home if you request it.  

The Minnesota Aging Pathways is a free, statewide service, staffed by licensed and trained health and human services professionals.  Minnesota Aging Pathways helps older Minnesotans and caregivers find answers and connect to the services and support they need. 

Help is available Monday through Friday from 8:00 AM to 4:30 PM by calling 800-333-2433. Providers can also refer people to the Minnesota Aging Pathways for help at the referral site.
