Minnesota Aging Pathways helps older Minnesotans and their caregivers find answers and connect to the services and support they need. We can help with Medicare, find prescription drug cost assistance, complete forms and applications, provide long-term care counseling, find resources in your community, connect people with caregiver support and much more.
