A qualified health care professional must complete a preadmission screening (PAS) prior to admission to any Medicaid-certified nursing facility.

This presentation is for professionals who want to learn more about how and when to submit a PAS to the Senior Linkage Line for nursing facility admission.

The Senior LinkAge Line can aslo provide resource coordination to help older adults return to a communtiy setting after a nursing home stay or help peole living at home who are at risk of losing their independence.
